This form may be used to establish or change your Tee Box Selection.
Your Tee Box selection is initially noted on the new member application. Open periods for change requests will occur in December and at Mid-year in June with the selection becoming effective in January and July.

The Board also recognizes that there are reasonable instances when a Tee choice may need to be updated during the year due to health or injuries. After submitting the completed form, the change request is transmitted to the Membership Chairman for review.  Requests outside the noted open time periods will also be submitted to the membership chairman for review and approval.
​Upon approval by the membership chairman, updates will be provided to the Pro Shop for updates to GolfGenius within 10 days.


                              Verbal requests for changes through the Pro Shop will not be accepted.
