As a preliminary step toward implementing the “new” ClubExpress® application, you are being asked to submit your limited personal data with this form. This information will be used exclusively for SCF Men's Golf Club activities and communications.
With submission of this form, you are simply providing data under the condition that it will be protected for use only in the ClubExpress® system and may not be externally sold or otherwise marketed to.
The ClubExpress® system is proposed to be used by all SCF Chartered Clubs and Special Interest groups. With submission of this form, you also agree to receive general purpose emails for SCF Charter Clubs communications that are essential to membership.
No access to ClubExpress® will be possible without your submittal of this form and approval as noted below. The Resident ID# will be used to confirm valid SCF residency.
If you terminate your membership in all SCF Chartered Clubs, your data will be deleted from the ClubExpress® system.
Upon entering the current date and submission of this form, you are approving the foregoing. Your input will be digitally saved, and date-time stamped.
